Skilled Nurses

Skilled nurses are assigned to patients based on their specialty and the type of treatment needed. They provide expert care and education for managing acute or chronic conditions. Expertise may include:

  • Intravenous therapy
  • Wound and ostomy care
  • Chemotherapy
  • Injections
  • Ventilator support, management and education
  • Urinary catheter care
  • Medication management and education
  • Home safety assessments
  • Follow-up care for mother and new baby
  • Feeding therapy and nutritional guidance
  • Physical, occupational and speech therapist

The experienced rehab team provides services that help patients regain mobility and speech function to improve quality of everyday life. The focus of therapy may include:

  • Patient fall safety
  • Exercises for strength
  • Pain management
  • Communication techniques
  • Assessing safety related to swallowing
  • Maintaining current functional levels as long as possible

Medical Social Workers

Social, emotional and financial problems may prevent patients from receiving the full benefit of treatment. Medical social workers help patients work through these issues.

Home Health Aides

Under the supervision of skilled nurses and therapists, home health aides work with the patient and family to provide personal care in a compassionate and understanding manner consistent with the goals and needs of the patient.

Dietitians

Dietitians conduct nutritional assessments and recommend an appropriate nourishment plan while providing education.
